Tamoxifen Tablet 20 mg
1. WHAT IS IT?
Tamoxifen is an oral medication that belongs to a class of drugs called Selective Estrogen Receptor Modulators (SERMs). The “20 mg” refers to the strength of the active ingredient in each tablet. It’s often prescribed daily for several years (commonly 5 to 10 years) for its long-term effects.
2. NATURE (Pharmacology and Mechanism of Action)
- Classification: Selective Estrogen Receptor Modulator (SERM).
- How it Works:
- In Breast Cancer Cells: Tamoxifen acts as an anti-estrogen. It binds to estrogen receptors in breast cancer cells, blocking estrogen (a female hormone) from attaching to these receptors. Since many breast cancers rely on estrogen to grow, blocking this action helps to slow down or stop the growth and multiplication of these cancer cells.
- In Other Tissues: In certain other tissues, such as bone, tamoxifen can have estrogen-like effects. This can be beneficial (e.g., helping to maintain bone density in postmenopausal women) but also contributes to some of its side effects in other organs like the uterus.
- Metabolism: It’s metabolized in the liver into active compounds (like endoxifen), which contribute significantly to its therapeutic effects.
3. ADVANTAGES
- Established Efficacy in Breast Cancer: Tamoxifen has a long history of proven effectiveness in treating and preventing the recurrence of hormone receptor-positive breast cancer.
- Reduces Recurrence Risk: Significantly lowers the chance of breast cancer returning after primary treatment (surgery, radiation, chemotherapy).
- Risk Reduction: Used to reduce the risk of developing breast cancer in high-risk individuals (e.g., strong family history, certain genetic mutations).
- Applicability: Effective in both premenopausal and postmenopausal women, as well as in men with hormone receptor-positive breast cancer. This is a key differentiator from aromatase inhibitors, which are typically for postmenopausal women only.
- Bone Health Benefit: In postmenopausal women, it can help prevent bone loss, acting somewhat like estrogen on bones.
- Oral Convenience: Easy to take daily as a tablet.
4. USES (Common Indications for 20 mg strength)
- Adjuvant Treatment of Early Breast Cancer: Used after initial treatments (like surgery, chemotherapy, radiation) for hormone receptor-positive breast cancer to reduce the risk of it coming back.
- Treatment of Advanced/Metastatic Breast Cancer: For hormone receptor-positive breast cancer that has spread to other parts of the body.
- Ductal Carcinoma In Situ (DCIS): To reduce the risk of invasive breast cancer after surgery and radiation for DCIS.
- Breast Cancer Risk Reduction: For women identified as being at high risk for developing breast cancer.
- Off-label Uses: Sometimes used off-label for conditions like male gynecomastia (enlargement of breast tissue in men) or certain types of infertility (though its use for infertility is restricted in some regions due to safety concerns).
5. PRECAUTIONS
Tamoxifen is a powerful medication with significant side effects that require careful monitoring and management.
- Blood Clots (Venous Thromboembolism – VTE):
- Increased Risk: Tamoxifen significantly increases the risk of serious blood clots in the legs (Deep Vein Thrombosis – DVT) and lungs (Pulmonary Embolism – PE), which can be life-threatening.
- Symptoms to watch for: Sudden chest pain, shortness of breath, sudden cough, pain/swelling/warmth in one leg. Seek immediate medical attention if these occur.
- Risk factors: History of blood clots, prolonged immobility (e.g., after surgery), obesity, older age.
- Uterine Cancer (Endometrial Cancer/Sarcoma):
- Increased Risk: Tamoxifen’s estrogen-like effect on the uterus increases the risk of developing uterine cancer, particularly with longer duration of use and in postmenopausal women.
- Symptoms to watch for: Any unusual vaginal bleeding or discharge (especially in postmenopausal women), pelvic pain or pressure. Report these to your doctor immediately.
- Stroke:
- Increased Risk: There is an increased risk of stroke, particularly in postmenopausal women.
- Symptoms to watch for: Sudden numbness or weakness (especially on one side of the body), severe headache, slurred speech, vision problems, balance issues. Seek emergency medical attention if these occur.
- Pregnancy and Breastfeeding:
- Contraindicated: Absolutely contraindicated during pregnancy due to potential fetal harm.
- Contraception: Women of childbearing potential must use effective non-hormonal contraception (e.g., condoms, diaphragm) during treatment and for at least 2 months (or longer, up to 9 months, as advised by your doctor due to its long half-life) after stopping tamoxifen.
- Breastfeeding is not recommended.
- Liver Problems: Can cause liver enzyme elevations, fatty liver, and rarely, more severe liver damage. Liver function tests may be monitored. Report symptoms like unusual fatigue, jaundice (yellowing skin/eyes), dark urine.
- Eye Problems: Can cause cataracts or retinal changes, leading to blurred vision or other visual disturbances. Regular eye exams are advisable.
- Drug Interactions:
- Antidepressants (CYP2D6 inhibitors): Certain antidepressants (e.g., paroxetine, fluoxetine, bupropion) can significantly reduce tamoxifen’s effectiveness by interfering with its metabolism into its active form (endoxifen). Always inform your doctor about ALL medications (prescription, OTC), supplements, and herbal products you are taking.
- Warfarin (blood thinner): Can increase the effect of warfarin, leading to increased bleeding risk. Close monitoring of INR is essential.
- Grapefruit Juice: Avoid or limit grapefruit juice as it can interfere with tamoxifen metabolism.
- Menopausal Symptoms: Common side effects include hot flashes, night sweats, vaginal dryness, and mood changes.
- Bone Pain/Tumor Flare: Some patients may experience a temporary increase in bone or tumor pain when starting tamoxifen.
- Ovarian Cysts: Can occur in premenopausal women.

- Store Tamoxifen tablets at room temperature (typically 20°C to 25°C or 68°F to 77°F).
