Mycophenolate Mofetil Tablet 250 mg
1. DESCRIPTION / WHAT IS IT?
Mycophenolate mofetil Tablet 250 mg is an oral medication containing 250 milligrams of the active ingredient, mycophenolate mofetil (MMF).2 It is commonly found as a capsule or tablet, though the 250 mg strength is often associated with capsules.3 MMF belongs to a class of medicines known as immunosuppressive agents or antimetabolites.4
It is a prodrug, meaning it is inactive when consumed but is rapidly converted in the body to its active form, mycophenolic acid (MPA).5 MPA is the compound responsible for the drug’s therapeutic effects.
2. NATURE (Pharmacology and Mechanism of Action)
- Classification: Immunosuppressant, Antimetabolite (specifically, a purine synthesis inhibitor).6
- Mechanism of Action: Mycophenolate mofetil works by selectively inhibiting the proliferation of lymphocytes (a type of white blood cell, specifically T and B cells), which are key players in the immune response.7
- Once converted to mycophenolic acid (MPA) in the body, it acts as a selective, uncompetitive, and reversible inhibitor of inosine monophosphate dehydrogenase (IMPDH).8
- IMPDH is a crucial enzyme in the de novo pathway of purine synthesis. Lymphocytes heavily rely on this de novo pathway for their rapid proliferation, unlike other cells in the body that can use a “salvage pathway” for purine synthesis.
- By inhibiting IMPDH, MPA depletes the guanosine nucleotides (building blocks of DNA and RNA) specifically in proliferating T and B lymphocytes. This leads to the inhibition of DNA synthesis and arrests the replication of these immune cells.9
- This targeted action makes MMF highly effective in suppressing the immune response that leads to organ rejection, while having less impact on other rapidly dividing cells in the body compared to some other immunosuppressants.
3. ADVANTAGES
- Potent Immunosuppression: Highly effective in preventing acute organ rejection, significantly improving the success rates and long-term survival of transplanted organs (kidney, heart, liver).
- Targeted Action: Primarily targets the proliferation of lymphocytes, reducing the overall immune response while potentially having fewer side effects on other rapidly dividing cells compared to older immunosuppressants.
- Steroid-Sparing Effect: Often used in combination with other immunosuppressants (like cyclosporine or tacrolimus) and corticosteroids.10 MMF can allow for the reduction or even elimination of corticosteroid doses over time, thereby minimizing steroid-related side effects.11
- Versatile Use: Effective in preventing rejection of various solid organ transplants.12 It is also increasingly used off-label for several severe autoimmune conditions where its immunosuppressive properties are beneficial.13
- Oral Administration: Provides convenience for long-term daily management by patients at home.
4. USES (Indications)
Mycophenolate mofetil Tablet 250 mg is primarily used:
- Organ Transplant Rejection Prophylaxis: Used in combination with other immunosuppressants (such as cyclosporine or tacrolimus, and corticosteroids) to prevent the body’s immune system from rejecting a transplanted organ (kidney, heart, or liver).14 It helps the body accept the new organ by suppressing the immune response that would otherwise attack and damage it.15
- Autoimmune Diseases (Off-label or specific indications): Increasingly used to manage severe autoimmune conditions where the immune system mistakenly attacks the body’s own tissues.16 While not all uses are FDA-approved, common off-label uses include:
- Lupus Nephritis (kidney inflammation associated with Systemic Lupus Erythematosus)17
- Other forms of Systemic Lupus Erythematosus
- Certain types of vasculitis
- Inflammatory Bowel Disease (Crohn’s disease and Ulcerative Colitis)18
- Pemphigus and Bullous Pemphigoid (blistering skin disorders)19
- Myasthenia Gravis (in some cases)20
5. STORAGE
- Store Mycophenolate mofetil tablets at room temperature, generally between 20°C to 25°C (68°F to 77°F).
- Keep the tablets in their original container, tightly closed, to protect them from moisture and light.21
- Do not crush, chew, or break the tablets/capsules, as direct contact with the powder can be harmful (e.g., irritation to skin, eyes, or mucous membranes if inhaled). If a tablet or capsule is broken, handle it with care (e.g., wearing gloves) and avoid direct contact with the powder.22
- Keep out of reach of children and pets.
